Role of the 15 mg dose
Top of the ladder. 15 mg is the highest licensed strength of tirzepatide in the UK. In SURMOUNT-1 it delivered an average 22.5 percent body weight reduction over 72 weeks. We supply 15 mg to patients who have worked through the ladder under our supervision and continue to benefit from treatment.
How long you stay on 15 mg
Open-ended. Patients can remain on 15 mg as long as the pharmacist judges the benefit outweighs any risk and BMI remains in the treatment range. A planned step-down is built into the long-term plan once weight goals are met.
What to expect
- Powerful appetite suppression — many patients report needing to consciously remember to eat
- Continued loss in patients who plateaued on 12.5 mg, although the rate is usually slower than at lower doses
- Side effects from earlier doses are largely gone by the time you reach 15 mg
- Improvements in blood pressure, blood sugar and cholesterol that often happen alongside weight loss
Injection technique
Once a week, rotate sites, same time of day if possible. Keep pens refrigerated. After first use a pen can be stored at room temperature up to 30 degrees Celsius and must be used within 30 days.
Pharmacist review
Monthly pharmacist review remains in place at 15 mg — there is no point on the ladder at which supervision stops. We track everything and have an honest conversation about whether to continue, step down, or pause.
When to seek urgent advice
At 15 mg the absolute size of any side effect is largest, even though the rate of new side effects is low. Anyone developing severe abdominal pain, persistent vomiting, signs of pancreatitis or gallbladder disease should stop treatment and contact NHS 111 or 999 immediately.
