Role of the 2.5 mg dose
Tolerance dose. The 2.5 mg pen is not designed to produce significant weight loss on its own. Its only job is to introduce tirzepatide to the body gently so that the gut, liver and brain can adjust before the first true treatment dose at 5 mg.
How long you stay on 2.5 mg
Four weeks, no exceptions. Even if you tolerate 2.5 mg perfectly from day one we will not let you step up before the four-week mark — that is how the licence is written and that is how the side effect profile stays manageable later in the ladder.
What to expect
- A small amount of nausea in the first 48 hours after the first injection, easing by day three or four
- Reduced appetite within the first week — many patients in Urmston tell us they forget to snack
- Mild changes in bowel habit, usually loosening then settling
- Modest weight loss of one to three pounds across the four weeks is normal but not guaranteed
Injection technique
Inject once a week on the same day each week into the front of the thigh, the back of the upper arm or the abdomen. Rotate the site weekly. Pens are kept in the fridge until the day of use and can sit out at room temperature for up to 30 minutes before injecting to reduce sting.
Pharmacist review
Book a four-week review with our pharmacist before your fourth pen runs out. At that review we decide together whether to step up to 5 mg or hold at 2.5 mg for one more month.
