Role of the 12.5 mg dose
Step before the maximum. 12.5 mg is used when 10 mg has plateaued and BMI remains in the treatment range. It gives one more bump in appetite suppression without going straight to the top dose.
How long you stay on 12.5 mg
Four weeks minimum. Often used as a stepping stone to 15 mg rather than as long-term maintenance, but a small number of patients settle here.
What to expect
- Mild return of nausea or early fullness for a few days after the increase
- Renewed loss after a plateau on 10 mg
- More dramatic reduction in portion size at meals
- Some patients notice tiredness in the first week after stepping up

When to seek urgent advice
If you are losing more than two and a half pounds a week for several weeks running, the pharmacist may recommend stepping down rather than up — sustained rapid loss can be unsafe.
